Docket® is commissioned and directly endorsed by the following official government entities: the New Jersey Department of Health and the Utah Department of Health.

New Jersey and Utah residents with a valid phone number or email address on file with the New Jersey or Utah immunization registry can access digital immunization records with the Docket® app.

NOTE: Docket® only supports access to COVID-19 records for New Jersey residents.

Use of Docket® is entirely optional.
