The iHealth Unified Care platform helps empower diabetes and hypertension patients to better manage their health and get real-time feedback from their primary care physician and care team members. iHealth Unified Care provides patients with a customized care plan, real-time chatting capabilities with a care team, and Bluetooth connected blood glucose and blood pressure devices.
