My Premise Health is a secure mobile app that puts your health information in your hands. Real-time appointment scheduling, view lab results, check vitals, message your provider, manage medications, mobile video visits, complete forms and more from one place.

With My Premise Health you can:

It’s never been easier to get, stay and be well.

To use the app and access your health information, you must first create an account through your healthcare provider or have an active My Premise Health account.

Don’t have an account yet? Contact your health center directly or sign up at www.mypremisehealth.com and click “sign up now”.
